Where to start
Well to start exploring I would say you need to have an efficient clone lol. Exploring EVE is definitely my favorite thing to do. But I have to tell you sometimes there are rocks that really really do not want to be turned over. And in EVE some of the critters under there are more than deadly.
Aside from planning to die I would make sure you have a few skills trained such as the scanning skills. These are more than necessary when it comes to having fun while exploring the universe. Do the tutorial that focuses on exploration and get Astrometric Rangefinding trained to level 3 at least before you even start. This will ensure you can get in and out of a wormhole within a decent amount of time.
Cloaking is another necessary skill to have trained.... I think it needs no explanation.
Hacking and Archeology are the money making skills for explorers.

Each Race has their own scanning frigate and I would recommend starting there. They are inexpensive and they get the job done. These ships should have a bonus to the scan strength of your probes. They also have enough high and medium slots to allow you to equip a cloaking device and a hacking module or an archeology module.

Aside from finding cool places, meeting terrible people, dying, and taking pretty pictures there are other reasons people go exploring.
Scan down a site and get started
Exploration sites can be broken down into the following types:
Cosmic anomaly
Cosmic anomalies are simple encounters in open space with various waves of NPC pirate ships, rogue drones or sleeper drones that players can kill. These sites sometimes contains a faction spawn or have a tiny chance of triggering an escalation into DED complexes.
Radar
Radar sites are also known as hacking sites, which require the player to utilize the Codebreaker module to hack the sealed cans found in those sites. Radar sites are usually lightly guarded by some NPC pirates and can be cleaned without much effort. Typical loots from radar sites are decryptors, datacores, interface blueprints, interface components and invention-related skillbooks.
Magnetometric
Archeology or salvaging sites are picked up as magnetometric signatures by the probes. These sites contain ship wrecks that can be analyzed by analyzer or salvaged by salvager. These sites are guarded by local NPC pirates or rogue drones. One can acquire T2 rig blueprint copies and T1/T2 salvaged parts from these sites, it should also be noted that analyzing the wrecks can sometimes trigger booby-trap and damage the scanning ship.
Ladar
Harvestable gas clouds emit ladar signals. These sites are rare in New Eden and contain gas cloud or fullerites that can by harvested by gas harvesting modules. Booster-related items are said to be found in ladar sites as well. Rarely there can be a pirate base inside the gas cloud with hackers containers.
Gravimetric
Gravimetric sites are hidden asteroid belts containing ores of rare types than those commonly found in the system. Do beware that NPC pirates also visit those hidden belts regularly to attack unarmed mining vessels.
Unknown
Unknown sites are either combat-oriented sites or wormholes. These combat sites does not require special skills or equipments to acquire the goods; however, the pirates found in these sites are much stronger and the players will have to use a properly fitted combat ships or sometimes a fleet to finish them.
DED complex
DED complexes are sites rated by DED usually consisting of several deadspace areas connected by acceleration gates. Those sites vary in difficulty and the harder ones contains very nasty structures that can do massive damage to player ships. A commander or structure can be found in the last room that may drop deadspace items and faction loots upon destruction. Those sites are not known to have any escalations.
Unrated complex
Not all combat sites have been rated by DED, some are Unrated Complexes. The destruction of certain ships or structures may trigger an expedition, which guides the player through several places to further investigate the incident. Commanders can sometimes be seen in those sites and killed for faction loots.
Skills Depending on the site scanned down, there are several skills required in order to gain access to the rewards stashed there.

- Archaeology — Archaeology I allows use of Analyzer I for opening otherwise-unopenable cans in magnetometric sites. Archaeology V allows use of a Analyzer II. Archaeologists are a special breed of explorers with skills and equipment specific to handling archaeological sites.
